Ordering InformationPhysician Stamps for RequisitionsCurrent and accurate physician information such as phone and fax numbers are fundamental to ensure accurate report distribution and/or the communication of critical results or priority requests. To ensure that all patient requisitions have current, legible information for referring physicians, CLS provides all of its physicians with a rubber stamp. The stamp contains the physician's surname and first name, the address of the health care provider and two unique identifiers, a client number and a provider number. These identifiers facilitate the delivery of reports to the correct address. The provider number distinguishes office locations by a unique physician number and an alpha suffix. Physicians having more than one office location will be assigned a provider number with consecutive alpha suffixes. For example, Location 1: 123456A; Location 2: 123456B. Please be aware that this stamp is unique for each office location and should only be used at the location it was delivered to. Please check the address label on the envelope that the stamp arrived in to ensure the delivery was made to the correct address, as reports will be delivered to that same address. For physicians to obtain a stamp (new or replacement) or to update fax or phone numbers, please print and complete the Provider/Client Information Form (CLS Form #OP7052) and fax it to Data Maintenance 770-3235. The laboratory information will be updated from this form submission and a new rubber stamp will be delivered to you. Please be sure to return all “outdated” stamps to CLS via your regular courier run. If for some reason you have two stamps at one office location, ensure that only the most current one is available for use. Telephone inquiries can be made to Data Maintenance at 770-3612. Specifying a "Copy to" PhysicianWhen requesting additional report copies please provide the first and last name of the 'copy to' physician or the name and fax number for the intended fax 'copy to' physician. If an office location is not provided for the 'copy to' physician, and he/she has multiple locations, the report will be directed to a default location for that physician. Repeat DeterminationsShould you question the validity of any test performed at this laboratory, it will be investigated and the analysis repeated. By contacting LIC at 770-3600, repeats can be conducted on the existing specimen. If stability of the specific component is questionable, a repeat patient collection may be required. In this case, please indicate “repeat determination” on the laboratory requisition. | ||||||||||||||||||||||||||
