Transfusion Medicine Requisition

Requests for pretransfusion testing and blood products must be processed on a Transfusion Medicine Pretransfusion Testing Requisition- REQ9004TM (TM2028) form for all sites where a Hospital Information System is not used, during downtime when the Hospital Information System is not available and Mobile collection services.

For blood product provision, the following information is required:

• Type of product
• Number of units or volume of product
• Date of intended transfusion
• Site/ facility of intended transfusion
