Requisition

Specimen Handling and Transport

Collect fresh tissue no larger than 3 mm3. Place immediately in a 4 ml screw-cap vial containing sufficient 2.5% Gluteraldehyde to cover the specimen. Label the vial according to CLS requirements. The time the specimen was place in Gluteraldehyde MUST be indicated on the requisition. This is critical information. Separate sample sites must be submitted in separate vials. If orientation is required it must be noted on the Requisition.

Transport the specimen at room temperature to Anatomic Pathology, 11th floor, Foothills Medical Centre.

If the specimen is taken after hours, it can be kept at room temperature until the next working day.

Notes

Check the expiry date on the vial of gluteraldehyde before obtaining the tissue.  Gluteraldehyde is available Monday - Friday through the Hospital site Histology Labs (Physican's offices may call 210-8806).
