1 x 5 mL gold top SST tube

2 mL serum

Collect specimen approximately 2 hours after loading dose, or 6 - 12 hours after start of maintenance dose.

CLS staff: collect specimen and forward to DSC Referrals who will obtain approval for testing.

PSC: Centrifuge and transport serum on ice to DSC.
RRL & extra-regional: Centrifuge, pour off, and freeze serum immediately, then transport to DSC.

Complete therapeutic drug monitoring information, including time of last dose on requisition.
