Usually accompanies a request for a CBC, however unstained air dried blood films may be submitted accompanied by Hematological data (complete CBC) and patient history.
Patient clinical information and/or clinical history will ensure that a pathologist will examine the smear.

Extra Slide Requests:
In cases where an extra blood smear/slide is requested for specific Hematologists:
DO NOT order a BLS; order test by using the "Extra slide" mnemonic linked with the appropriate physician's initials (ES + physician's initials). See list below.

Apply bar-code smear label to empty tube, attach tube with elastic band to patient's EDTA tube.
