Peripheral Blood Smear, Extra Slide Requests

Mnemonic Extra Slide Requests:
In cases where an extra blood smear/slide is requested for specific Hematologists, order test by using the "Extra slide" mnemonic linked with the appropriate physician's initials (ES + physician's initials). See list below.

Apply barcode smear label to empty tube, attach tube with elastic band to patient's EDTA tube.

Note: For other extra slide requests (no physician mnemonic available), order as BLS, make a patient level non-chartable comment "Extra slide for Dr. (enter name)".
