How do I get copies of all my lab results sent to a new physician?

There may be additional costs for a complete records search.  It is preferable for the consulting physician to contact CLS directly at 403.770.3600 to confirm which records are required.

How do I get copies of my lab results sent to my Naturopath?

CLS does not release results to Naturopaths.  To obtain copies of your lab results, complete the Request to Access Health Information form.
