The biopsies, LEEP and ECC specimens must be placed in separate formalin containers, 1 SPECIMEN PER CONTAINER. All the specimens must be sent to the laboratory together. Containers must be securely tightened to eliminate any leakage.
Label the containers according to CLS requirements. The containers must be labeled with the patient's full first and last name and at least one unique identifier such as Personal Health Number (PHN), Unique Lifetime Identifier (ULI), Personal Identification Number (Federal, Military, RCMP, Refugee, Immigration, Passport), Facility Assigned Number (RHRN, MRN, ACB #, Clinic #, Chart #). The containers must also be labeled with the exact specimen site orientation (e.g. 6 o'clock, 9 o'clock).
