Specimens MUST be accompanied by a copy of the original requisition and completed Hemostasis Studies History Form SC4900. Send the original requisition for optical scanning. Note: for lab-collected SCM orders on in-patients, only history form is required, not requisition.

Note:
If patient has received Desmopressin or Factor Concentrate, indicate medication, amount received, time of infusion/injection, IV or subcutaneous, patient weight.
Additional Information Note: Bleeding time and platelet aggregation studies must be booked with Special Coagulation. Closure time is collected Monday to Friday 07:00-13:00. No collection on statutory holidays.

For stat testing on weekends or off hours, clinician MUST contact the Hematopathologist on Call to obtain approval. Samples can be collected but test will not be processed unless approved.

If a stat PT/PTT is required at ACH, draw one additional 1.8 mL draw plastic tube and indicate "Stat PT/PTT" on the requisition.
If unable to draw additional tube, submit all short-draw 2.7 mL tubes to Special Coagulation; indicate "Stat PT/PTT" on requisition. In this case the tests will be performed off-site and the turnaround time for results may be extended.
