Transfusion Medicine Requisition

Requests for pretransfusion testing and blood products must be processed on a Transfusion Medicine Pretransfusion Testing Requisition- REQ9004TM (TM2028) form for all sites where a Hospital Information System is not used, during downtime when the Hospital Information System is not available, and for Mobile Collection Service (MCS).

For blood product provision, the following information is required:

• Type of product
• Number of units or volume of product
• Date of intended transfusion
• Site/ facility of intended transfusion
