Completing the Pretransfusion Testing Requisition

Requests for pretransfusion testing must be processed on a Pretransfusion Testing Requisition- REQ9004TM (TM2028) for all PCUs where a Hospital Information System is not used, during downtime when the Hospital Information System is not available, and for Mobile Collection Service (MCS), and must be used in conjunction with the Regional Transfusion Identification System (RTSIS) form (CLS0997 or TM2199).
